Billing Policy and Fee schedule

I bill for my time with a patient and on behalf of a patient. The time is reserved for you.
In additon this may include consultations, writing reports or discussions that are needed for your care or that you request.

Fees for individual or couple psychotherapy or consultations: $300-450
depending on type and length of session

Group medical psychotherapy is $125


If I cannot provide the services you need at a rate you can afford,
I will be happy to recommend alternatives for you to consider.

Do you accept insurance? How does insurance work?


I no longer participate in insurance company networks even though I may still be listed in a directory.
However, my services may be covered in part by your health insurance plan for " out of network providers ".
I have listed specific questions to help you find out what your benefits are.

If you do have out of network benefits you will need to apply for reimbursement for therapy fees yourself after payment.
I will provide you with the forms to file a claim with your insurance company. However i cannot guarantee this
reimbursement.


Our contract for service is between you and me directly

Payment for the initial consultation is due at that meeting.

For regular therapy sessions I can bill monthly. Payment is due upon receipt of the bill

Questions for insurance compamies

To determine if you have mental health coverage, the first thing you should do is check with your insurance carrier.
This can be difficult. But do not give up even if you have to call another representative.
Please check your coverage carefully by asking the following questions:

• Do I have mental health benefits?
• Does my plan permit coverage for an out of network provider M.D. psychiatrist?
• Do I need preauthorization?
• Is approval required from my primary care physician?
• What are the dollar amount that your company considers to be the usual and customary fees for psychiatric treatment (CPT code 90834) or (90836 and 99212) .
• What percentage of those amounts does my plan cover for individual medical psychotherapy (CPT code 90834) or (90836 and 99212)with a board certified MD psychiatrist
• What is my deductible and has it been met? Is there a separate deductible for out of network provider benefits
• How many sessions per calendar year does my plan cover?
• Do I have a maximum limit on number of sessions or benefit cap ?

What if I cannot come to an appointment. ?

Since I reserve and schedule time specifically for you, I do bill for the time you had reserved.
unless i reeive 48 hrs notice that you will not be able to keep the appointment.
